It has been significantly updated, and is now available as a research monograph, published by Wiley (May, 2004).

I've also begun work on a new book, tentatively entitled "Numerical Sound Synthesis," which is under contract with Wiley, and due for publication in 2008. I will periodically place updated copies of the draft, again in .html format, here.
